Starting point is 00:01:38 product oh yeah that's what i was thinking i mean does bone talks count or like an actual product i like that answer because that would be all of ours yeah okay well taking away that that okay let's do like regular product okay um i know mine okay i talked about it i think it was one of our other questions my dr lancer eyelash serum is like 150 bucks but it's the best thing is it you like it ever you want to try it oh yeah i mean because you'll get like another one of those whatever off of amazon but it's it's worth it got the money all right what about you kelty god so many i'm such a product whore you know what i mean like just a whore in general um i think probably like all my face creams i use like all of them no i use like a set
Starting point is 00:02:24 10 step system from like i use all a little bit of kate somerville a little bit of obagi a little bit of mura like i use like one of every product like a serum those are all but i will say i do use the giorgio armani sheer foundation as my foundation and it's like super luxurious and I think it's like $75 a bottle. Starting point is 00:02:47 We should try it. That's the most expensive product ever. We're not that famous, Jack. I mean, I do have like 63,000 Instagram followers. I think mine is that SkinCeutical. Well, it's not really that expensive. SkinCeuticals? Yeah.

Okay, so, you know, a couple weeks ago, we were talking about, like, the throwback Good Week, Bad Weeks, you know, the vintage moment. Oh, yeah, yeah, yeah. And mine is so good that I'm bringing it out.
Starting point is 00:03:55 What is it? This isn't against the rules, Kelty. It is, but we're going to continue, because I kind of like the idea of bringing back... Bad moments from life. No, I agree. Yeah, yeah, yeah. Oh, but I can't do it now, because I need to think. Yeah, you need to think.
Starting point is 00:04:05 Okay, so, she is... What? Kelty Knight. Oh. the character in this story kelty knight this is gonna be a long one grade 11 11th grade for people who live in america yes i had a really hot boyfriend named kevin duff kev my mom used to call him dr duff because he wanted to go to medical school and i'm sure that she wanted me to marry him is he a doctor now no um anyway do you know no he's not he just like bike he went
Starting point is 00:04:35 on a bike ride through africa or something he's like he's cool of a doctor no he's very cool doctors without borders doctor yeah so anyway okay but that's not what this is about i was a complete loser growing up and i like had all bad loser boyfriends but this guy was like a cook at the restaurant i was a waitress at and he was so handsome and he was popular and it was like i had cha-ching did he take your virginity no okay and anyway so i was really excited about dating him and on so he had a weird family situation his parents were divorced and the mom and dad then had gotten back together so high school kevin whoever let this happen had his own house and then the mom and dad had reconvened at the townhouse so really they didn't want to
Starting point is 00:05:15 divorce each other they wanted to divorce a child i guess so it was like this summer and we were working at the restaurant and then like he asked me out on date and this is our second date and it was a daytime date this is what happened i was at the time in a ballet summer school and i had been doing like point work for like eight hours a day and so i had had this like really really bruised big toenail and i was so embarrassed about it because it was summer and we're wearing like bare feet that i put like opaque silver nail polish on all my toenails to cover it up so that you wouldn't know that my toes were like completely beat to shit right so then we're like sitting this is so amazing we're sitting on the floor and we're like having a makeout and then we like break from
Starting point is 00:05:57 the makeout and I looked down to the left and my entire toenail, the whole toenail had pulled out of my toe and it was just the skin part underneath my toe and the place where my toenail should be. And it was just sitting on the Berber carpet. Did he see it? No. And then I didn't know what to do because it was Kevin and he was so hot and popular. And so I quickly picked it up and I shoved it back on my toe.
Starting point is 00:06:22 Did it stay? This hurts. It stayed for a minute. And then I like tried to leave. i like i hate feet so much that like i i get hot like about people can you imagine making out with like your dream guy and then like your toenail falls off like what do you say sorry about my toenail like now i'm a mutant did you later paint the skin it's opaque silver well now you can get like a nice no maybe you could i've seen it on instagram you could gel but it was before the
Starting point is 00:06:46 time of gel anyway okay we're done with that okay good week no more of those i had my destiny cards read by a destiny card reader do you know what that is is this different than just a psychic yes or so they go off sounds like a bunch of bullshit no it's very interesting they go off they said first of all they go off your birthday and then you each each birthday has a card and then you have corresponding cards and why they work but the first thing she said to me she grabbed my hand and she goes i know you feel like everyone is slow but they're not you're just incredibly fast do you think it's because you were so pushy no that's what she's that's my card i was super chill like now is it because you emailed her 15 times can we talk about how
Starting point is 00:07:32 kelty came in today she's like i'm so chill today aren't i really chill we're like kelty if you have to say it i don't think so can i finish my story jack no yeah so anyway the destiny card reader also told me a little bit about my husband chris's card and he is the nine of clubs and the night for his birthday april 25th and the nine of clubs is the sexiest card in the deck sexiest yes he's very sexual oh sexual the most sexual he's just the sexiest card it's like dominating powerful like a real man loves pleasures his like big flaws he can get lost in the pleasures of the flesh that makes me want to throw up i forgot to tell you this i just That whole thing was horrible.
Starting point is 00:08:15 So every single person, not every single person, that's an exaggeration. So many people say things to me along the lines of, oh, well, Kelty's husband's British, right? Everyone thinks that Chris is British. He looks very British. He looks so British. My friend Emily calls him the Brit. Yeah. Because she's like, he's the Brit.
Starting point is 00:08:33 I think it's because he has this like wacky hair. Wacky hair. He's tall and real thin. And then he has like kind of like. He's got a British face. He doesn't have like veneered teeth like everyone else in Hollywood. like he's like a regular smile i don't think he had braces you know what i mean yeah anyway okay who's next i'll go go ahead okay um my bad week and i had to be careful because when i was thinking
Starting point is 00:08:54 about this i thought i could offend someone but it's not shocker it's not going to i am throwing a baby shower for one of my best friends she's having her first baby and i i volunteered to do this because i don't live in the same city as her and i felt like this is the contribution that i can make because i can't be there to hold your baby for you nor do i want to because i really hate holding newborns but we'll get back to that um so throwing a shower might be on the list of top two things i hate most there's only two bachelorette party more no but on the top of the list okay it's really it's really hard i'm sure have you ever done it in what way yes you have yes well here's the thing i was thinking about my own baby shower what if i ever did this and i was
Starting point is 00:09:43 thinking of 10 people that i would have there because that's just i'm anti-social my friend is very southern no she's not she's from new york so she's but she's much nicer than i am there's so many people the guest list is 65 people oh my god what i'm essentially 65 people I'm essentially throwing a wedding for an unborn baby. So who's helping you pay for it? So that was the other thing. I really struggle with money situations. So if it had been the 10 people in my mind, and not 10, I was thinking 25, I was just
Starting point is 00:10:16 going to pay for it and help everybody, whatever, and have everybody help and make suggestions or whatever, like not financially. But then when I got the guest list of 65 people, I was sitting there and I was like, this is awkward. So now... It's going to be 10 grand at least. that's in new york city so now so then i like reached out to the sisters and the family members who wanted to help her anyways and but anyways it's just a disaster and i i fucking hate dealing
Starting point is 00:10:39 with money and so that was that's my bad week yeah that sucks but what games are you playing we're not playing games because she told me she would kill me so maybe we should i love some of those games i love the game the best game i don't i've i've been the diaper is not no you should definitely play the baby name game we did with britney port that was so fun where you do like your first and last day people well everyone it'll probably oh yeah you can't really yeah you can't really play games with that many people that's a lot of people anyways you know what you could do is plaster her baby belly i did that once plaster it's really cute you can have a plaster artist come in no and they like plaster and then you all paint it no it's gonna be so
Starting point is 00:11:17 ugly a plaster who what pregnant woman wants to put their belly in plaster it's like our friend holly madison did it it's like paper she's holly madison she's a playmate of course she put her belly in plaster she put her vagina in plaster before that okay i love you holly um my my good week is my i've been coming in with a lot of quotes lately from housewives and things like that 24 karat twat well my mother actually gave me the greatest quote but it came through my dad's mother who's no longer with us she once said to my mother you either cook in the kitchen or you cook in the bedroom or you're gonna lose your man and i'm currently doing none of you me too i'm also on a nun diet it's like not it's the opposite of of cooking it's like
Starting point is 00:12:10 dying you're just cooling down you're cooling down those slowly slowly decompose put it all in the freezer cold cuts in the bedroom decomposing food okay jack um i really like that thank you that's amazing um your message alert that is a mirror yes um okay so my bad week i'm gonna start with i was out this weekend at a bar and really what jack huh you guys it's really weird what i'm trying this new thing of like going out drinking drinking it seems so strange it's good for you good for you you know you're new new and improved jack i know jack 3.0 so i'm out i meet this really really hot guy right super tall obviously blonde really funny and we just like randomly start chatting having like really good conversation
Starting point is 00:13:01 like really good chemistry yada yada yada and then we both noticed that we never exchanged names so i'm like i have no idea what this guy's name is jack and i wish that is my biggest goal in life is to date another jack i was really hoping for you no he his name is i'm gonna call him john and he's like oh my name is john i'm like oh i'm jack and i like go to reach out for my handshake i am a very like i have a strong handshake yeah the guy does the over overturned wrist and just took my hand with his little his little his little fingers nope and my vagina has never gotten so dry so fast in my entire life and then he started talking about how he was that it for you and then he start talk starts talking about how he wakes and bakes and i'm like gotta
Starting point is 00:13:46 go oh this is done i'm sorry never no never again you guys and he probably thought it was cool when he was saying it's like yeah i love to wake and bake and he was like yeah well he was actually talking about microdosing weed but i was like all this is bad people that brag about smoking weed as adults are the people who started smoking weed at like 28 and i have news for you if you didn't start by then you should have never started it's not hot it's not like it's not hot it's not hot to be a stoner at 35 i'm sorry it's not attractive you are gonna be a stoner just keep it quiet guys yeah secret our little secret nobody needs to know no one knows maybe he was so stoned that's why his hand was limp he lost control but then you slept with him no i can't i can't get past it
Starting point is 00:14:27 there's a lot of things i can look past yeah i mean i've dated i love that you like yeah i love that you dated spider-man on hollywood boulevard but you won't let the guy who wake and bake he did that guy was very he's a very big muscle well yeah because he's collecting the dollar bills what was your good week uh that's like so that's my deal breaker is a little can shake um my good week so anybody who listens to the podcast knows that i got my bike stolen a couple weeks ago and i've got to say that the lady gang is so amazing that one of our lady gang members in our facebook group messaged me well actually she posted in the lady gang and she's like jack i saw you got your bike stolen my husband's company owns a bike company and i want

Starting point is 00:18:13 guest today is a board certified otolarginologic cyst we're gonna have her say that in a minute and facial plastic surgeon located in pasadena california she's an ear nose and throat specialist and also an expert in cosmetic services like non-surgical injections and fillers lasers and this is going to blow your mind she also performs radio frequency surges for snoring which I didn't even know existed, but I think a lot of lady gang members are about to sign up their husbands right now. Please welcome to the lady gang. Please don't judge me on the pronunciation of this. Dr. K. DeRay Raj. That was perfect. It was. Okay. But now help me out with otolaryngology. Okay. Otolaryngology. And that is head and neck surgery in simple terms.
Starting point is 00:18:57 Here's my question. When you were a little girl, you're like, I can't wait to grow up and be an otolaryngologist? Well, actually, my dad is a head and neck surgeon, otolaryngologist too, so that's kind of where I got my inspiration. Did you know that that existed as a job? No. Well, I specialize in everything
Starting point is 00:19:14 from the clavicles to the top of your head. That means like facelift, neck lift, eyelid, all the good stuff. Is it so crazy when you facelift and you pull someone's face off and then you have to like... Do you pull the entire face off? Yes, I trained at UCLA
Starting point is 00:19:29 And that's where we do major intense head and neck cases where, really, you take a person's head apart and put it back together. So that's my interest in reconstructive facial plastic surgery. That's so interesting. What's the craziest thing that's gone through your mind when someone's face is off? It's just very surreal because, literally, you can take a person apart and put them back together and make them whole again in a lot of different ways. Because some people suffer from, you know, they're born with facial disfigurement. But you really reconstruct the person. Wow.
Starting point is 00:19:58 I'm very happy about the work you do, but we're vain. So we just want to know about fillers and Botox. But I also, I did go deep on your site and you do a lot of incredible work that really does change people's lives. Like our Jack Vanek. Yes. So what we did is we have a secret Facebook group. If you guys are not in it, please head over to Facebook.
Starting point is 00:20:18 And we basically asked the girls, what do you want to know? And there's everything from. There were like hundreds of comments. Really? Oh my God. And so I have narrowed them down and some of them are surgical, some of them are non-surgical. And so just feel free to give us your real advice, what you really think. We're trusting you.
Starting point is 00:20:36 So don't fill us with like the cell-o-vision. Tell us what really works and what doesn't. Okay? Promise? For sure. All the whole truth. Let's start. Okay.
Starting point is 00:20:43 The first question is a Botox question. She says, I've been wondering about Botox. I've heard some physicians say that the earlier, the better. But a doctor I know and trust actually says the opposite. that Botox paralyzes the muscle. When you don't use it, it atrophies over time, thereby creating long-term problems. So essentially, you look nice for a while,
Starting point is 00:21:02 but not so much in the long run. Her suggestion is not to stay away from Botox at all, but just not to start too early. What is it? Well, my personal philosophy is you need to prevent wrinkles. Your skin is like a piece of paper. The more you fold it, crinkle it, the more you unfold it, it'll be crinkled and wrinkled.
Starting point is 00:21:19 So why have that in the first place? So I'm a big believer in a small amount of prevention starting at an early age, as early as your 20s. As soon as you see that you have those lines, you can already tell where your mom is, where your grandma is, where your face is headed. So what's the point of waiting? But if you do Botox, I have a big forehead. I have a five head. If you do it over and over and over again, which I do, over time, will my eyebrows start drooping if I'm not using this muscle all the time? No.
Starting point is 00:21:47 And the atrophy of those muscles is actually a good thing because you don't need frown muscles. You don't need squint muscles. You don't need muscles that wrinkle your nose like a bunny. Those muscles, it's good if they thin out and they get disused because then over time, you don't even need Botox as much. So I started Botoxing in my early 30s. I never even got the lines, but even in between my Botox, it stays smooth. And no, you're not going to get a heavy brow or brow ptosis. Really, the sagging and drooping changes we get are related to skin elasticity.
Starting point is 00:22:17 So I like to think of skin like your bathing suit, like you pull it out of your drawer, and some summers it's great and springy, and then suddenly the elasticity goes, and it's like, what happened to that pair of underwear or that bikini bottom? Your whole bush is hanging out at the pool. That's right. Like that loss of stretchy elasticity is what makes this thing sag. So Botox is not the problem. Okay. Next, nose jobs. People want to know about the real recovery time.
Starting point is 00:22:42 So there is a lot of recovery time for nose jobs. 80% of swelling goes down in the first three weeks. The remaining 20% can take 6 to 12 months. And the nose really settles and shifts and remodels, a lot like getting a house done. Things look great the first few weeks, your contractor signs off, and then two months later, little door frames are shifting off, and scarring is happening, and swelling goes down. But if it looks good at three weeks, that's really how it's going to stay,
Starting point is 00:23:07 and it's only going to get better. Did anybody ask about the filler for the nose job? Yes, people did. Do you want to talk about it now? Let's talk about it now. So people want to know about the non-surgical nose job. So that's fabulous, and it's so popular in my practice. And I really want to counsel you guys to make sure you don't go to just anybody, any old nurse injector who can do your filler in your nose, because the nose is the danger zone of the face. In case you didn't know that, there's a million blood vessels coming into it.
Starting point is 00:23:33 They don't have valves. They communicate directly with your brain. What? Yes. So not to scare you, but please seek a certified professional, a board-certified surgeon, ideally. Because I take the nose apart. I know where all these vessels are, and we know how to prevent problems there. So what happens if they inject it in the wrong spot and you hit one of the vessels?
Starting point is 00:23:52 Well, there's lots of case reports of, sadly, blindness, loss of vision, or stroke. Yeah, so. But in the millions of treatments. No, but here, we've talked about this a lot. No one has ever given us a warning. Thank you, Dr. K. Isn't it the same with Kybella? And I didn't want to tell you this before you did it.
Starting point is 00:24:10 No, Kybella does not cause strokes, but there can be some nerve weakness. And all of these fillers are, let's not scare people. They're 100% extremely very safe. Otherwise, I would not do them on myself. I'm not crazy. But they are only safe in the hands of a qualified injector. So if you're going to see someone who's an esthetician or in some salon somewhere, please check out their work and check out their training and their credentials.
Starting point is 00:24:34 yeah it's scary i feel like a lot like yeah there's a lot of group on listen guys yes and don't trust it if it looks too good to be true it means they're giving you diluted crap you know i don't think you should shop for inexpensive when you're working on your face no you should not you only have one face and it shocks me how people are so particular about who does their hair color and their highlights yeah but they'll go to any old group on for their face right wow you can only get paralyzed and have a stroke but yeah okay that's just like a little small downside do you do chemical peels are you familiar okay i'm thinking about treating myself to a chemical peel next month and i want to know i'll come over and do a peel what do you what what do you
Starting point is 00:25:15 recommend for the ladies well i'm a big fan of chemical peels lasers get all the hype because they sound high tech and they look cool um but you know what chemical peels are safer they've been around longer and you can really grade it to the person's skin so you can get nice the the when you hear about the word peel you think you're raw red and well i think of samantha in sex in the city remember she got the peel and then she'd like walk around the party yeah yeah so the peels these days i use peels that are very high technology that are the ingredients get absorbed inward the outward skin does not slough off so you literally look pretty and fresh even you can go to work the second and third day a little mild flaking on the fourth and fifth day and the and
Starting point is 00:25:55 then you have just fresh, bright skin. And so what kind of peel do we ask for? Our women are all over the world. So when they're looking for, do you get the alpha hydroxy? Do you get the chemical? I don't know what it is. I like a light TCA peel. That's trichloroacetic acid. There's a couple different companies. There's a million companies that make peels. The idea is to see a person who looks at your skin, make sure they understand your pigmentation potential, because peels can also create a ton of pigment and brown spots. And you don't want that. And you don't want to stir up a lot of inflammation so you're going to start really light with like a six percent kind of tca peel that's my ideal and then you build from there and you always want to do a
Starting point is 00:26:33 series of treatments like three three series peel to get your skin to a good boot camp situation how long okay so a month apart yeah so when you say you can go to work like the next day sometimes people say that and then it's a big lie so can you really can i go to work are people going to know that I was effing with my face. Literally, no. Literally, I am right in someone's face giving them Botox and I have my peels and I do them. You guys should all get in the habit of doing them seasonally. So at the end of summer, get rid of the sun damage. At the end of winter, get rid of the dry skin. So about quarterly is perfect. I love that. Once you're in good shape. And if you've never done anything to your skin and you got lots of sun damage and brown spots,
Starting point is 00:27:11 then you're going to do one a month to reach that state of being great and then maintain with quarterly. How many of us are wearing a third love bra right now? I won't say who does it, but they, you Hollywood starlets get peeled and then they go live at the montage hotel for a month to recover. Oh yeah. Have you heard of this peel? Yes. So I'm talking deep peel. I'm not talking, you can go to work victim. Yes. I'm not talking, you go to work the next day. Can
Starting point is 00:28:36 you discuss those peels? Yes. So those are the older peels and those we did a lot. I didn't do them but then the 60s and 70s people did them a lot and those are like a phenol very deep layer of peel where you do need to be under a sedation or anesthesia for that and they're they're actually quite amazing for like the older population of patients like 60 to 70 year old ladies who really want to they almost work as well as a facelift because of the amount of tightening that you get and the wrinkle reduction but they do kind of make you lose a little pigment so we've all seen some Beverly Hills ladies that are a little too pale and too pulled and so it's that kind of thinking. So phenol peels are still done, but they're losing popularity because of the downtime.
Starting point is 00:29:15 Would you recommend somebody to do, like an older woman, to do one of those instead of a facelift? I would probably do a CO2 laser instead of a phenol peel because you get a lot of tightening effect and you take off three or four layers of the damaged skin. So if I was in my 60s and not wanting to do a big major facelift with a week of downtime, but you still get a week of downtime, But it's a fraction of the cost, and you also get brand new skin. Why do so many facelifts look bad? That's a great question. I think a lot of it has to do with training.
Starting point is 00:29:49 And the old philosophy of being pulled tight like Joan Crawford does not exist in new training. So we believe in re-volumizing the face. I believe in that really strongly. And I do that with facial fillers for people like us in our 30s and 40s and 50s. That's really appropriate to use a filler to build up the cheeks and prevent drooping and sagging. Once you have too much skin where you have to pull up in the mirror all the time, then you might start thinking about doing a little mini lift or a little mini tightening. So that's a good criteria.
Starting point is 00:30:18 I always ask patients, well, is it this you want when you pull up on your face in the mirror and you look great? Then you really need a facelift. You have too much skin and you need tightening. I could just try it. And the new version of facelifting is where we lift the deeper layers of the muscle. We rebuild with transplantation of fat where you need fat for volume. Facial fat is like gold. So you never want to lose your facial fat.
Starting point is 00:30:43 And then the minimal tightening and the hidden incisions. So that's like the new lifting. Where is it hidden? Behind the ear? It's in the preauricular crease, which is that little fold right behind your tragal area. Wow. And then it goes right behind your ear. Huh.
Starting point is 00:30:57 Yeah, into the hairline. Because you know on all facelifts you can see the whole line. Yeah. Yeah. People, one of our ladies wants to know, I have a major sun stash. How do I get rid of it? That's a good one. So why do you get a sun stash?
Starting point is 00:31:11 Do you know what it is? Anybody know? When I was on birth control, the pill, I got the weird pigmentation above my upper lip. Is it darker or lighter? Darker. It's melasma. So melasma comes from too much waxing from pregnancy and hormone changes. And sometimes threading will do it.
Starting point is 00:31:29 Anytime you're putting traction on the skin. So you got to be really careful if you're someone who gets waxed and you're starting to notice a little pigmentation, immediately start bleaching creams on that and then stop waxing. Instead, switch to electrolysis or something that's not going to create inflammation there. Okay. Nurse Jamie is another famous in your world. And she, one of the ladies wanted your opinion on the Uplift Roller.
Starting point is 00:31:55 So Kim Kardashian swears by it at $65. Does it actually help with anti-aging and de-puffing? or is it better to use a jade roller or some other beauty roller is this all hype can we say i don't even know what this is a roller like a face roller yeah so they it's like this little roller and it's got like kind of plasticky spikes on it and you're supposed to like a little rolling pin yeah i have one that's like um i keep in the freezer and it's ice yeah but it's not like the micro needling no no it's not going into your skin like a roller pin sounds like putting a band-aid on a bullet wound me yes exactly well you're not the expert today becca to me okay this
Starting point is 00:32:31 is my prediction for what she's about okay what does our expert think i think nurse jamie is like a brilliant marketing and um money-making person i'm really like excited to meet her one of these days so her devices are really interesting the idea of rolling your skin to take out lymphedema is wonderful. So yes, if you put ice on an area, if you use a cold roller, it's going to take away some swelling. It's going to give you some mild circulatory benefits and improvements. So there's no downside, but is it all that? I wouldn't pay $65 for a roller. No, sorry. Good. Not me. How do you get rid of whiteheads? How do you get rid of whiteheads? So what is a whitehead? It's a comedone. It's a plugged up pore and there's a layer of keratin and oils beneath that. So you
Starting point is 00:33:15 need to be on a regimen that helps your skin exfoliate regularly so if you're prone to those little bumps that just don't come out and you try to pop them out you guys should be using glycolic and salicylic acid so glycolic wash is amazing like my wash is really great for that and then a salicylic kind of a product which reduces bacteria retin-a of course is like the number one ingredient that i put all my 30 year olds on anybody 30 and above if you're not using retin-a please start using it do you have your own retin-a i do your own brand yeah what percent of because it's like a retin-a percentage right that you use what do you recommend like starting on well i always like to start on the smaller percentages like a 0.5 you know and there's a
Starting point is 00:33:58 lot of retin-a's out there on the market if you look at oil of olay and all these over-the-counter products they have retinol which is a version of retin-a it's not the true retin-a and i'm generally starting patients on a 0.5 percent and then we're working upwards so don't be fooled by the drugstore brands retinol go to the dermatologist and get the retin-a go to the dermatologist and get the retin-a yes and it's often covered by health insurance right right until after you're 30 that's what i've what no it's covered as an acne treatment when you're younger and then as you get older they don't care about aging when they don't care they don't care that you still have acne at 35 okay um next i've seen a few different types of injections for lips can she
Starting point is 00:34:36 explain them and how to choose the right one? Restylane, Juvederm, Volure? That's a very good question. And I get this question a lot. I think I do really a lot of lip injections. And let me just tell you that lip injections to do them well takes practice. So please see someone who's done a thousand lip injections. Don't be their number 10 injection. Okay. And then number two, be able to choose someone who you trust them to look at your anatomy and choose your filler for you. I'm like an artist when it comes to fillers. I have every filler available. I'm not going to be one of these brand name girls that goes with one company because I get the cut rate price when I buy bulk. So I have every filler and I look at your anatomy and I determine where you need
Starting point is 00:35:16 your volume and what level of support. So some of these fillers are quite thin, like Vobella, very thin. Some are quite thick and voluptuous, like Restylane is typically more voluptuous than Juvederm. And some of them are more long lasting, like Volure lasts 18 months. So all of them have their specific criteria so generally i'll tell somebody if you want really really natural we'll go with fobela it'll look like you almost didn't do anything a month later you'll want more if you want one step up from that maybe we go with juvederm and then if you want a little more voluptuous go with rustling those are sort of my basic recommendations huh i love that microneedling it's all the rage yes it's what the ipl was last year is microneedling of this year
Starting point is 00:35:59 I feel like it's so new, I'm afraid to try it just in case it's one of those things like 30 years from now that they find out is really bad for your skin. No, they'll never find that out. And it's been around for more than 30 years. And it's like an amazing treatment for your skin. It's like a supercharged facial. So microneedling is this process where we make little small micro injuries to the skin. You don't feel it. I use a needle roller.
Starting point is 00:36:21 It has like 550 titanium needles. And they're 3 millimeters long. So quite deep. It's not like these crappy ones you can buy on Amazon. so don't try go buy one don't do it at home okay yeah and yeah self micro needle sounds worse than a self bikini wax super unhygienic yeah very unhygienic so painful if you do it yourself at home but we use a really great like 23% lidocaine tetracaine mixture you won't feel it and as I roll that you're going to get all these little areas of micro injury and what how does your skin fix
Starting point is 00:36:51 an injury by making collagen right so here we go we made little openings in the skin skin's going to repair with a burst of collagen production. And now we have the benefit of nice products being able to seep into those holes. So stem cell growth factors will be infused. The high dose vitamins will be infused. And hyaluronic acid, which is a skin plumper, goes right under the surface of the skin. So there's no better treatment than microneedling. It's awesome. What's better, microneedling or a peel? Or are they just totally different? They're kind of different. So a peel is going to exfoliate and remove the top two or three layers of sun damage brown spots and tighten the pores and everything. Microneedling is going to
Starting point is 00:37:28 work on the inside of the skin to boost circulation and boost collagen production. It helps with fine lines, scarring, like it helps a lot with acne scarring. It helps with reducing pore size and it's just really great for you just get a glow. You do two or three microneedling, so it's not a one-time treatment. You will look good with one time, but if you can commit to doing that every couple months for three or four treatments just a meeting amazing transformation in your skin like no more problem skin i have a question for you i have psoriasis and oftentimes if i have a spot on my body that i continue to kind of like reopen the skin it freaks out by overcompensating to create more skin cells and that's the flakiness that you see i've always been scared to do too
Starting point is 00:38:14 much like the microneedling to my face because what if is that something you have to watch out for if you're like no not at all if you get psoriasis externally or you have you know dermatitis or any other skin conditions we'll look at you real carefully and we'll look at your previous spots but no it's not going to trigger that on your face good to know that's great yeah what's the average cost and what's the sort of average downtime can i do it on a friday and go to work on monday without anyone knowing i mess with my face yeah you're the very next morning you'll look just glowy and fresh you'll leave the office looking a little red you'll feel like your skin is all circulated like you had a good workout and you will look a little red but the very next day
Starting point is 00:38:53 you'll look amazing just minimal redness you can wear makeup and it costs like around 650 for a treatment in our office i want to motivate people to come in and do the follow-up treatment so it's only it's like 1500 for three treatments and oh great we send you home with the stem cell growth factors to use too cool fascia blasting so everyone knows i work out with my girlfriend and Rebecca every day and she just bought a fascia blaster and now she's all bruised and it's crazy so does it work I have a cheaper one from Amazon that's
Starting point is 00:39:22 made out of wood but uses the same technique well describe what that is it's like an intense roller for cellulite and you can use it on your butt yeah it definitely works so that's like endermology but the cheap version of it so have you guys heard of endermology
Starting point is 00:39:37 so endermology is where they do it's that big suctioning vacuum machine that clamps onto your skin and sucks the cellulite and then allows the puckering to settle better so it's similar to that so you're just kind of massaging tissues down into through those connective tissue bands so they're not so cellulite okay lady so it does work it works but you have to really do it you have to do it it's like in the 50s it was popular with past wives to use a rolling pin yeah that is coming back because it's so almost archaic and like the technological age that we're in yeah but yeah
Starting point is 00:40:11 Everybody says it works. What is your favorite cellulite go-to minimizer? Well, that's good that you asked. So right now I'm doing a lot of Sculptra injections because it's bathing suit season and Sculptra is one of these collagen fillers. So wherever you have a puckered area that's creating a cellulite dimple, it's super hard to get rid of those. The Selfina laser that they've touted to be like really great at that, it is effective. but it's so many treatments and it's bruising and downtime sculpture is easy because you're giving a shot right in the area of the depression and even like one treatment i'd say like 40
Starting point is 00:40:48 improvement in that area did you do it what do you think oh i loved it i like totally totally saw a difference yeah and i like i talked about cellulite yeah you hate it yeah i hate it so much yeah i love it but how long does it last well the nice thing sculpture creates your own collagen The product itself is there for two whole years, so that's nice longevity of the product. But the collagen you grow around the sculpture is yours for the next five to ten years. We don't really have studies that show how long, but I think at least five years. The skin on my thighs is less dimply and more just… Gross.
Starting point is 00:41:22 I'm just kidding. You're amazing. No, gross. You know when direct sunlight hits your thigh and it's that… That's like 2.30, it started happening to me. Like crepey-ish? Yeah. It's not quite cellulite.
Starting point is 00:41:31 But if I pull up on my thigh skin, it looks nice and smooth. Is that gravity? Is that cellulite? So that is that elasticity change. And then definitely using like a retinol-based body lotion will be a nice thing to do for that. So like, you know, I have that Titan Tone and Lift Lotion. Oh, yeah. And you see a difference even in just a few days.
Starting point is 00:41:51 Like it's crazy how you think you're going to be young forever and then suddenly you look down and you're like, why are my arms kind of creepy? And then so that changes when you put some retin-A on there. So would facet-blasting help with that, or does that only really help with the skin? I think the thigh skin is just kind of thinning and atrophying. So awful. Yippee. I love getting older. Or I could just stop wearing shorts.
Starting point is 00:42:13 Short shorts, buy back a Tobin. Okay, last one before we go to break. Will getting some Botox or some sort of injections under my eyes help with dark circles? I've had them my whole life and go through phases where I'm super self-conscious about them. So not Botox, but filler. Under eye filler is one of the most amazing treatments that we do. And it's an off-label use of the injectable fillers. I mean, it's not FDA approved, but I do so much of it.
Starting point is 00:42:38 And it's very popular now. And you can easily, with just two small little needle sticks, very, it's almost painless. But I can thread in some filler and it just literally photoshops away the darkness, the hollow, the veins. And you just look so refreshed. It's a really nice treatment. it. There's no downtime. I use a cannula, so it's a really nice technique that doesn't bruise you. I have a question again. I'm really hogging this session. I did, for the first time ever, an under eye filler. You did? I did. Wow. And I got one black eye that was there for
Starting point is 00:43:12 weeks. Three months. I'm so sorry. Wow. But here's my question. As he was injecting under the eye that got really bruised i felt like a pop and someone was saying the bubble and the solution could have sort of caused that but i'm so now i'm terrified i probably will never do it again and i don't know if i should if that was a fluke or if that's just i should never do it i think it's just technique dependent and that can be a fluke you can with the cannula i'd say 95 of my Patients don't have any bruising. And if anything, a little pinpoint bruise. Anyone, any injector, if you inject enough, you can cause a black eye.
Starting point is 00:43:53 Yeah. She looked like she got sucker punched. It was crazy. Yeah. It's just, that's unfortunate because it's such a great technique. I don't want you to be scared of it. I think if you come to the, come and see me and we'll use a cannula and you'll be good. Yeah.

Starting point is 00:44:24 know now you know i have been through many relationships with my under chin double chin of my 30s first we did cool sculpting which was early last year i talked about it on the podcast i did the mini cool sculptor thing and it sucked out like a vacuum and then they said this is going to get rid of all the fat and it's going to be gone i think it did nothing and then i paid a whole bunch of money to have tightening lasers done oh i didn't know you did that because they were like well you need the coolest company i'm sorry we forgot to tell you plus the tightening laser okay fine then i bought my own tightening laser on amazon which is not as powerful but if you use it every day they say if you use it 60 days straight and so every morning when i drove
Starting point is 00:45:06 to work i put the lotion on and i did it on my way to work for 15 minutes it did nothing so i took the jump and i did the kybella last week um i will tell you it hurts like a bitch uh you have to wear a thing around your head to like keep the swelling you know pressed against your body for like three days which really was great for my sex life um i did get bruised a little not too much bruising but your face blows up like you're a bullfrog like it really bullfrogs out when they say you get a bullfrog it's a full bullfrog i would be embarrassed to go to work i took a week off they said they said oh you can do it on friday and you can be back to work on monday that's a lie unless you live in like nebraska and you're wearing a turtleneck and your contour is on
Starting point is 00:45:48 television right i'm also on television but i will say even if i wasn't i would not want to go into my office because everyone can tell you did something to your face and i tried to think of all the lies i could tell people like oh i had a i got my feet like where did you get your face stuck anyway so now a vacuum i'm a week later it's still swollen it's numb and apparently now it begins to eat itself and soon i'll look like gg hadid what do you think dr k have the faith you're gonna look like bella am i yeah well she has a new face so it'll be fine yeah um most of what you said has some truth in it so does it sting it stings like a bee sting i would say if If you've ever been stung by a bee, that's kind of intense.
Starting point is 00:46:30 Except they put in like 30, like 15 or 30 needles at it, like 15 bee stings. So the first two to three minutes, and most patients say like a four or five out of 10 on the pain scale. I have a couple of patients say nine or 10. Last three days, everybody said like three. Not too bad. Those were men. Yes.
Starting point is 00:46:47 Yes. They were trying to tough it out, I guess. No, I was saying the men were saying it was a 10. Oh, the men were the 10s. No, the men were really good. So it helps if you do one vial your first time and just ease into it. You must have done probably two vials the first time. Because I only have this one week off, and then I have to go back to work.
Starting point is 00:47:02 So there's like no time to keep doing it. So there are ways to finesse your Kybella treatment. And let's just start by saying it does work. It's an enzyme. It melts fat. It's the enzyme that's found in your gallbladder. So that's what melts your dietary fat that you eat when you're having steak or whatever. So it does definitely work, and it's a very safe way to melt fat.
Starting point is 00:47:20 And pinpoint fat reduction that's permanent has never been done before. So it's kind of a game changer. And if you've always hated your neck and you don't have time for surgery and whatnot, this is a fast, easy thing to do. But, yes, it stings. You can handle it because it's worth it to get rid of your fat. It's definitely, like, not as bad as period cramps. Right. Like, if your worst period cramps, yeah.
Starting point is 00:47:40 I mean, it's annoying and it's tender, but it's not like you're going to be in the bathtub and throwing up from the pain. Yeah, yeah. Once your treatment's done, like, by the time you leave the office, that five out of ten pain is gone. And then you do have the swelling, for sure. So I'll tell patients one to two weeks of downtime with swelling. And yes, you look like you have a double chin. And that feature of your face that you hate is emphasized for a week. But like, but your friends, but other people don't really see it.
Starting point is 00:48:07 Like I didn't, you know, I've done patients and I see their chins and they look fine, but they themselves criticize themselves. Yeah. I was going to say, like, I don't think I would ever. I think you still look beautiful. Thanks. Yes. Definitely.
Starting point is 00:48:18 You'll swell for a week or two. So don't do it the week before your wedding. Give yourself some time. And then when will I start to like, when's it going to. So it takes a month to start to see the swelling go down, and the second treatment is where you see your results. So you've got to just buckle in to, like, it's a series of treatments. One treatment's not going to do it. It's not.
Starting point is 00:48:35 The average is two to six. And I say if you can pinch an inch, it's probably two to three treatments. If you can pinch more than an inch, probably three to four. Yeah. What are you pinching? What do you have? Pinch your inches. And also the fun thing about Kybella is I can treat the other areas.
Starting point is 00:48:51 I wonder if Kaibala is now or going to be approved for off-label uses, stomach, inner thighs, boob fat. You could do your vagina lips. She just did a vagina one. You did a vagina? Or like it was on her. The mons pubis, yes. Like that pubic region. Sometimes you're a little fatty right in there above the pubic bone.
Starting point is 00:49:08 So, yeah, I've done a lot of off-label use. So I'm one of the kind of pioneers of doing the off-label uses. I'm doing the bra fat, the back fat, the certain people come for stomach rolls that they just can't get rid of. We have people doing the banana roll, that little fold under your butt that bugs you. The pubic area is like the most novel thing. And then I've done inner thighs and then the shoulder blade area. So, you know, in the early 2000s. Yeah.
Starting point is 00:49:35 When you wanted a thigh gap. Here's my question. Thigh gap. In the early 2000s, it was like I just remember being in New York City and liposuction for the masses has sort of begun. And I had some fancy friends that had money and they went and got the inner thigh liposuction and the back liposuction. And then the fat came in other places after because it was like such an intense suck out. How do you control the Kybella so that it's a smooth suck out? Well, it's only going to take out fat exactly where you deposit it.
Starting point is 00:50:01 So we put on that little grid of a tattoo that's a temporary tattoo. And just exactly those spots is where you're going to get fat reduction. It doesn't spread beyond that. And fat can come back if you eat it back. But that's a lot of calories to eat back. But you're not going to have like a hole? You're not going to have a hole. It's very gradual.
Starting point is 00:50:18 You're not going to have like a random pocket of fat on your arm? No. So like each Kybella treatment, I think you could take down about 20% to 30% of the fat in an area. So that's pretty good for no surgery, no downtime, and just kind of – I need to do it. Yeah. I know the other nice area is the jowls. If you have –
Starting point is 00:50:32 You can do it on the jowls? Yes. And so that's definitely a skilled injector technique because you don't want to weaken any facial muscles. But you can do these heavier jowls right here. But then you're going to look crazy when it's – That must be good for like older people. Yes. It's like it's great for like 50 to 60-year-old age where you're starting to get this little heaviness.
Starting point is 00:50:51 Yeah, because sometimes you can lift the cheeks. We can do a lot of treatments, but that's an area that's stubborn fat that won't go with anything else. Right. Wow. When our friend Cara Santana was here, she talked about using injectables to get her jawline to be sharper. Yes. Did you see my last Instagram? I did a couple posts on like jawline sculpting.
Starting point is 00:51:09 Tell me about it. It's so popular. I see a lot of models and, you know, it's a hidden secret in the industry that they all get everything done. and that's how they look fabulous but um so yes the jawline injections i use radius which is a calcium hydroxyapatite filler and that's um you know your job is your bones are made of calcium so why not supplement that with a little calcium and it's popular because it creates that sharp angle gives you that neck definition it just looks chiseled and elegant and looks really makes the face look good what happens when you're in your 20s or 30s and you get those like
Starting point is 00:51:44 one to three gigantic period zits. What are we supposed to do? Well, try not to squeeze the hell out of them because they never pop. You just, like, work on them and create more inflammation and pigment. Do get a topical antibiotic if you can, and benzoyl peroxide will be beneficial. And then, of course, come in and get a Kenalog shot. Come to the dermatologist and get a shot. How much are those usually, like, around $100?
Starting point is 00:52:07 No, they're not bad. I don't charge that much. I charge, like, $50. Okay. um why do i get the worst acne of my life after being at the beach is it the spf is it it maybe you have a little mild rosacea tendency there's some forms of rosacea if you're a person who flushes pretty easily and gets redness there's a little bit of acne related to rosacea that happens when you're out in the heat in the sun oh that must be it yeah so some protection for
Starting point is 00:52:33 it critical to use some protection but i'm using tons of it are you so then we also just need to look at your whole regimen like see what's causing the irritation and if it's your sunblock are you using a mineral sunblock it's literally every time i use sunscreen so which one are you doing i've tried all of them you got to try mine okay it's mineral mineral and the other one i really like so any sunscreen that's mineral because it's going to not plug your pores and sunlight hits the zinc oxide that's in it and it bounces right off you like all the other sunscreens the Neutrogena, the Coppertones, the whatnots, the SPF 70, 100, full of chemicals. You know, your skin has to absorb a chemical, ultraviolet, then hits your skin, and you
Starting point is 00:53:13 get this chemical reaction in your skin that neutralizes skin cancer-producing ultraviolet rays. But why have all that happen? Why not have the sun hit you and bounce off you? Right. It makes a lot more sense. Yeah. So mineral is best for acne-prone.
Starting point is 00:53:26 Got it. Yeah. Let's talk about stretch marks, baby. Let's talk about them on me. Stretch marks, is there anything we can do? Mama's after the baby. Maybe you have them from when you grew tall as a baby. Becca doesn't have that.
Starting point is 00:53:40 She never grew tall. Those are such the bane of our existence. But yes, so retin-A is known to improve stretch marks. So if you've had post-pregnancy stretch marks and you're not breastfeeding anymore, you definitely can get a prescription, one that you use nightly. Okay. And then lasers, there are multiple lasers that help with stretch marks, but it's going to be multiple treatments.
Starting point is 00:54:00 There is no magic bullet. Otherwise, we would all be doing it. And Sculptra, too, is great for just giving the skin kind of that taut, nice, smooth appearance where you don't see tiger stripes as much. Over-the-counter Differin versus Prescription. Oh, good one. Is there a difference? So Adelapine is the generic name. It's now sold as Differin.
Starting point is 00:54:21 It's a great product. It's nice that it's gone over-the-counter. Nobody knows about it. Nobody's doing it. But the proactive people, I think, are starting to use that. So it's great. It's a good product. And what do we use it for?
Starting point is 00:54:32 It's for acne, right? It's for acne. It's one of these retinols that's in the retinoid family, and it matures the oil glands so that your glands don't plug up. When you're young and immature, you have these twisty, curvy oil glands that don't know how to drain to the surface. And Retin-A matures them for you. So they'll drain nicely. They look foliates properly. And your acne gets worse the first month you're on any of these Retin-A products.
Starting point is 00:54:55 And then the second and third month, it gets better. after three months you'll be great and you'll maintain it off and on for a year very last question and then i have some goodies okay we have time yes um our girls kind of wanted to talk about reasonable botox like options so they're basically saying you know i know i need to be doing this it's not in my budget but like what exactly should i be asking for how much is too little to even make a difference you know like what do you recommend i think you ought to have enough faith in your injector to let them recommend to you because i get some women who come in and they're like oh i tell them they need 20 units that's the fda approved amount for your
Starting point is 00:55:35 forehead or your glabella area and then they'll come back to me with like i only need seven well like i don't know what to do with that like it's seven units is like giving you a dose of saline with a little touch of hope but you know so not really sure what to do with that so that's $50 for your hope. Yeah. So I think a safe, um, amount of Botox is you have to look at your brow. If you have a big brow, you're going to need more Botox. If you're a man, you need more than if you're a woman, the average woman I'll do like 20 to 30 units for their brow. And it's going to be about 10 to 15 units around each eye. And that is very legitimate, appropriate FDA approved recommendations. And then there's all kinds of deviations along that spectrum. There's all kinds
Starting point is 00:56:17 of people who want holistic Botox because they're organic and vegan and they want it to work but they don't want to feel too bad about it and I'm okay with that and I just give them a little lighter dose and then there's people who want to be frozen and then they want a really heavy dose and there's the spectrum
Starting point is 00:56:32 and I think you need to have some expression there's too much of the frozen going around so it's very independent on the person So your advice is save up it costs and if you want to do it go to someone good get what you pay for. It's so worth it.
Starting point is 00:56:48 Botox is like the one thing that's worth it. If you think about all the creams and stuff you've bought at the drugstore over the years and how many hundreds of dollars that is, nothing will work better. Nothing works better. It's pretty much a home run for all my patients and like once you've tried it you'll see the results. And there are
Starting point is 00:57:04 different, you can see like a super injector who is going to give you extra great results because it is possible to get Botox that simultaneously lifts your brow, opens your eye and gives you a wide, pretty, beautiful expression and it's also possible to get Botox that makes you heavy and you don't like it and it's just like what happened so you need to be careful before I went to her I don't know if you guys
Starting point is 00:57:24 ever noticed I remembered the guy that I went to for Botox before you put way too much in and it made my brows so so heavy that I couldn't even like see myself putting on makeup I'd have to like tilt my head back yeah but your technique is so good because I still can move my eyebrows yeah then it's I like to be really natural that's my goal with myself and all I think for all women And to be natural, maintained, and, like, just age gracefully and not be overdone. Starting point is 00:59:40 But it doesn't matter for me. But does it actually make your eyes darker? No. I've heard that. So, no. That's written on the product insert. If you put the drop in your eye, like a glaucoma drop, it can turn your eye color. But don't put your eyeliner brush in your eye.
Starting point is 00:59:56 That's it. There's no case reports of color change. Some strong advice. Yeah. And it takes about two months to see your full longer, darker, thicker. But they'll be double their length. Amazing. It's pretty amazing.

Starting point is 01:00:20 This is one of the newer forms of Botox and it's ultra pure. So the original brand name Botox actually has lots of protein in it, which if you've been on Botox for a long time, you might start to notice that you're not as responsive or it wears off faster.
