Welcome back to the strictly anonymous podcast. How are you today? I'm great. Thanks for having me, Kathy. No, thanks for coming back. My audience loved you. Uh, I think because, listen, you, do you have your own podcast? I mean, listen, you're a urologist. You're the real deal. You practice in Nashville. You do, though, also have your own podcast and YouTube channel called Balls Deep.

So, like, you're really good at, you know, giving regular people the information. You know what I'm saying? So that's why, I mean, my audience loved your first episode. I'll put the number of your first episode where we talked about a lot of different things, including ED. This time, we're going to mainly focus on vasectomies, correct? Yes. First off, thank you for the kind words, Kathy.

I really appreciate that. And, you know, yes, my goal is to always just put things in just straightforward, simple, you know, too many doctors are just stuffy and, you know, talk, talking this medical jargon. Like, just talk to someone like they're a real person. So, yeah, so today I thought, you know, let's talk vasectomies because, listen, I imagine most of your sexually active listeners here. They don't want their lifestyle changed by an oops, whether it be due to with their spouse or a different partner. No, but the other thing too that I got a lot of questions about when I put the word out that we were going to talk about the vasectomies is a lot of them, you know, yeah, they want to get one, but they're afraid because of side effects. Now, you do. There are two kinds of ways to do a vasectomy, right? Like a traditional way and no scalpel way, correct? And you do one of those. Yeah, I don't know if you could necessarily put it just into two buckets. You know, there's a lot of ways. There's a lot of ways to cut the tubes there, But yeah, I offer no needle, no scalpel, and I get guys high on laughing gas, and we get it all done in one visit.

So I think all those things together make our practice in Nashville at Pizone MD pretty unique. Wait a second. A regular vasectomy or one of the other kinds of vasectomies takes longer than one visit? Yeah, most churologists in the country, and don't ask me why. First off, they make you wait weeks to months just to get. in for the consultation. Right. Then after the consultation, they make you come back a different day to do the procedure.

And I just think that's ridiculous. So one thing we did with our practice, you know, you imagine I tell this to 5,000 guys over a couple of years and it gets a little monotonous. So what I did was I took every question I've ever been asked. Yeah. I made a video and I threw it on our website. So most of our guys by the time they find me because we get great ratings and good Google search is they've gone to our website.

they've watched all the videos other questions are answered kathy so essentially they've had the consult before they even come in and then it's just oh it's you like you look like your videos that's great those are awesome and we just BS a little bit about you know how many kids you have or don't have what you like doing and i do a quick exam to make sure i can find the tubes and then we just rock and roll you know there's no reason this needs to be there's no reason this needs to be anything less than simple Yeah, because I remember picking up a guy from his vasectomy appointment, so I'm assuming he had to go under, because why else would I, why else would I have had to pick him up, right? So there's none of that that goes down with your procedure, I'm assuming. Yeah, zero. So there certainly are people who undergo either IV sedation or general anesthesia. It's totally unnecessary. So most guys, they're going to come in for one visit. We're going to answer whatever questions. I'm I'm going to do an exam to make sure I can find the tubes and make sure that there's no other abnormalities, address any concerns.

And then after we get started on the laughing gas, which the guy controls on his own, you know, we got his favorite, you know, Pandora or Spotify station at Rockin. And in usually six to seven minutes, I'm done. And then the laughing gas wears off so quickly, guys can drive themselves home. All right, I have a million questions, and I don't know where to start. First of all, I haven't had laughing gas since I was probably like eight from the dentist, right? So I don't even know. Like, is laughing gas take away the pain? Does it just make you high that you just don't know what the fuck's going on?

Like, is that going to make you just not feel anything? And I'm wondering maybe it doesn't even matter because if you're not using a scalpel, like, I don't know what you're even doing if there is any pain. So those are like two questions. Yeah, absolutely. We could dive into the procedure. But let's start with the laughing gas. So it affects every guy differently. What I tell guys is this.

90% of guys choose the laughing gas when they come see me. And almost every guy is glad that he did. Because I can't ever predict, Kathy, what a guy's experience is going to be while I'm tugging and pulling? And that although, yes, I'm still doing a minimally invasive approach. We're still going to inject numbing medicine. Any guy who's been flicked or kicked in the nuts knows it's not a comfortable feeling. It's not a comfortable feeling.

Yeah. And the experience varies. I mean, I do this on guys with zero laughing gas. And they go, was that it? Like, is that what guys make a big deal about? Why do guys need the gas? And then I have other guys to use the laughing gas and go, oh, my God, thank God. I had that.

I think it would have been miserable without it. And it's a combination. You know, it's certainly, number one, it gives a guy something to do, right? Distraction is very powerful. Number two, yes, guys get high or giggly again to varying degrees. Some guys are out cold snoring. others can get actually a little bit more anxious and we've got to slow down on it. Most guys, I tell them they're at that like two to three beer buzz.

You kind of don't really care what's going on. You're in a good mood. Sure, there's parts that are mildly uncomfortable, but it just takes that edge off. And certainly for the guy who walks in and he's just a nervous wreck, it's a must. Yeah, well, okay, quick question about the procedure. Are you using a laser? Like, I mean, if you're using a scalpel and they do need a little, a little bit of laughing ass because maybe there is pain.

Like what is actually happening? Yeah, so let's walk through kind of how we do it. So the guy's laying there. He is undressed from the waist down. I got him covered up. So the only thing hanging out or, you know, his sack. And I'm going to find the tube. It feels almost like a mouse cord.

It's got a very distinct feel. So if any of your listeners are interested, you could certainly practice on yourself finding the tube. And that's about how uncomfortable it'll be. Where is the tube, though? Is it like on the bottom of your balls? Like by the like where the shaft meets the balls? Yeah.

So it is part of what's called the spermatic cord, Kathy. It's the tube of structures by which the nuts dangle from. And within that would be all the blood vessels, the vasdeferins, muscles, and nerves. And so it's above the balls just to the side of the shaft. Okay. And so if you feel it, you can feel a. almost like this mouse cord. I pinch it up against the skin.

And then I have a device that injects local anesthesia without a needle. It's kind of like a power injector. It's a spring-loaded device that can shoot the liquid anesthesia through this tiny little opening, almost like a spray gun. And it is so powerful, it goes through the skin into the Vazdefran's and numbs a guy up without a needle poke. So that's the no needle part. Is that something new? Do they use that for women when they give like the OSHA and stuff? Like, do they use that a lot? Because I've never heard of something like that. That's like pretty

amazing. It's pretty underutilized and it's sad, Kathy. It's not new. I mean, I've been using this technique for 17 plus years. And so it's just, you know, it's one of those things that some, some docs just don't get comfortable doing it this way. I love it and the patients love it. So, yeah, so that's the device that's the no needle part. Then once they're not, I'm going to use a sharp little instrument. It's called a hemistad. It's just got kind of like a pokey end. And I'm just going to make a little skin poke.

I'm just going to poke through the skin, spread it. It's maybe three to five millimeters. I mean, it's really not a big incision. But rather than cut someone open with a blade, which not only can hurt more but causes more trauma, this is more like a little poke in a spread. And it allows me when I'm done, I don't even have to put stitches into the skin that just heals up on its own. Okay. So, yeah, so once a little incision is made, then I'm going to grasp the Vaz deference, the tube I have pinched, gently pull it out of the body so I can see it.

And then I'm just teasing away any little layers around it. It's kind of like an onion. You know, you got a lot of different layers through, and you've got to get down to the right layer, peel off any of the little blood vessels. And then once you've got nothing but the tube, we cut it, we cauterize it, and I put a suture. and to sew the two ends away from themselves. So I do three different things to make sure this is a permanent solution. And then once I'm done on the left side, I let it retract back inside the sack, and I'm able to typically do the right side through the same incision.

So it's one tiny little skin poke, and I'm done in typically under 10 minutes. Okay, now this is where I'm like really dumb, because I don't even know if I've had sex with the guy that had a mastectomy. And so to know, like, what the difference is. Like, okay, so if you're carterizing that thing. and so nothing's going through it. Like there still is going to be some sort of ejaculate, right? But it doesn't have sperm in it.

Like there's still a liquid that comes out. But the stuff that gets you pregnant, like the sperm isn't in there. Am I wrong or right? I don't know. No, you're 100% correct, Kathy. And that's just a big myth. Guys go, well, I don't want to have a bisectomy because I don't want to,

bisectomy because I don't want to, I want to still cuff. Right. You still will have or something coming out, right? So where is that stuff coming out? Like, how do you, if you're like shutting down the area where the sperm comes, how does the liquid come? So is it always that the liquid is coming from a different place in the sperm anyway? Exactly. So 95, 95. 9.5. to 99% of a man's ejaculate is going to come from a combination of what's called the seminal vesicles and the prostate. And that's deep in the pelvis back by the bladder. So it is nowhere near where we're doing the vasectomy. The Vastephyrins, its only job is to carry sperm, which are the swimmers that lead to babies.

Yeah. From the testicles to what I call the swimming pool, which is the seminal vescles. So now that tube is very long. It runs from, you know, the balls up along the shaft, the penis, up into the groin, goes inside your body, down behind your bladder and to your prostate. So think of it like it's a 200-mile road, but at mile marker 25, I'm going to put up a roadblock on both sides. So the swimmers can't get to the swimming pool. But your swimming pool will still be full.

And the common ejaclet that comes out, you might. won't notice any difference. You can't tell that a guy's head of a second. All right. Well, listen, I learned a lot about my body when I started to do, when I was like doing some infertility treatments because, and I was like, really? I like, I didn't realize that I was like born with all my eggs. Like literally, I thought I just made new ones every year. I mean, I think we're very like, we just don't have the education to know about what the fuck is going on. So I'm just wondering like with guys, I know for a fact that they are producing new sperm all the time, right? So there's still from, it sounds like from what you said, like there's

still will be producing this sperm, but it's just like backing up or staying there? Or like, what happens to the sperm that's there that can't come out? It's a really interesting question that we don't know with 100% certainty. So you are correct. How could you guys not figure this out? Well, if we could fly in a fucking rocket ship, but we don't know where sperm goes? Right. So here's what we do know.

You are correct that men in most cases are going to continue making sperm to the day they die. Now, when you have a vasectomy, again, those sperm are going to run into the roadblock. And the part we don't know with certainty is, how does the body get rid of the sperm? Because what I can say unequivocally is that when we do vasectomies, I don't have guys come back and see me a couple years later with giant nuts. Like, it's all like there's not this backup of fluid. Yeah, guys walk out with elephant balls. Yeah. Yeah. So, so, you know, the sperm are the smallest cells. in our body. They're simply just our DNA and nothing else. So what we think occurs is they travel through this microscopic super highway in the testicle, outside the testicle into a structure called

the epididymus, which sits right behind the testicle, goes through part of the bas deference, and then they hit that roadblock or brick wall that I created. We think then the body's immune system probably reabsorbs those cells and cleans them up, you know, just like, you know, our stomach lining is always shedding and changing. Our skin cells, all our organs outside most of our nervous system are changing all the time. And yet we don't, you know, flake off these giant limbs or anything. So we think the same thing happens. And it's remarkable how in 99% of guys, there's no buildup of back pressure or fluid that leads to any issue. So most of us are bodies just adapt.

We're very adaptable species. And then if you have, if you wait long enough, certainly a number of men will stop producing sperm because there is a back pressure that can impact the sperm production. And we know that, Kathy, because some guys get a vasectomy reversed. Yeah, that's all every guy like. Yeah. Yeah, we can dive into that. But when we do the vasectomy reversals, there's times we don't find any sperm because it's been, say, 15 years since the guy had the vasectomy.

Wow. And again, because it's literally these almost millions of miles of these super tubes, superhighway tubes the sperm are running through. There was probably another blockage that happened, just like if you had a plumbing back up, enough back pressure and you're going to have multiple leaks, right? it's not going to just be in that one spot because that pressure. That is fascinating. It's so interesting. Is there any thing where like when guys get older, they produce less sperm or are they producing the same, like forget about vasectomies?

Or guys just like producing the same amount of sperm their whole life? Or does nature say let's produce a lot of sperm in these years because they're young and this is when they're going to have fucking babies? Do you know what I'm saying? Like, you know, just like nature for women stops giving us eggs at a certain time where we're not supposed to be moms anymore. Yeah, there's no doubt that sperm quality and or numbers decline with age. They do.

Certainly. Yeah. Yeah, absolutely. But, you know, I mean, who was it? Wasn't there some celebrity recently? Oh, yeah, right? Rod Stewart. I mean, they're all still doing it.

Yeah. Yeah. Okay. So it is reversed. Well, real quick about the procedure. How long does it take? So how long does your procedure take? Like, it sounds like minutes.

Yeah. Yeah, I mean, I've done it as quickly as in like five to six minutes. Wow. Usually it's under 10 minutes. Now, there are certain guys who have really hard tubes to find. And very often, these men are obese. They have a lot of fatty tissue around the cord. You know, I'm sure, you know, any listener can relate to either in themselves or a guy partner they've had.

You know, some guys have droopy nuts and some guys are high and tight. And the guys who are real high and tight and you're like, where's your ball sack? It just kind of looks like those little bulbs there. Those guys can be pretty tough. But, you know, 99.9% of the time I get it done the office, done in less than 10 minutes, all in the same day. And then, you know, I think an important question that people ask is, well, what's the recovery going to be like? Yeah. And, you know, it's, you know, we do these on Friday, Kathy.

By Monday, you're back to work. You know, it's not. I tell guys, give it a week. I've had guys have, I've had guys have sex. earlier, even masturbating. And it's just uncomfortable enough, just because when you have an orgasm, all those muscles down there are spasming. That's why sex feels so good. And so just give your body that week. And I'd rather that first sexual experience be delayed a couple days and it feels back to normal. Then you did it too soon and it's uncomfortable. So it's a week for sex and a week for any sort

of real heavy physical activity, you know, weightlifting, running, riding a bike. You know, You have a real physical job. You might want to take that next week off. But for most guys, especially desk jobs, you're back by Monday. And, okay, what's the difference between the time of recovery, you know, in one like that you do and a typical one that maybe, you know, you have to go in and they do use a scalpel and it's a lot more involved? You know, it really, it varies from guy to guy. I would say that in general, you have most of my patients, Kathy by Monday are good. I take all my own phone calls and listen, we can all admit as guys, we're not good patients.

We don't do pain well. I rarely get phone calls. I mean, it is really darn rare that guys are like, oh, my God. Now, I certainly anecdotally, and that's all I have is anecdotes because I've never done it the other way. Have guys who, you know, have guys who share their experience from other doctors and it sounds like it's a lot worse. Now, there's also guys who fly through it, even if they use a scalpel or a needle. But the way I look at, you know, surgery is if I can do something in a less invasive way, that's a better patient experience, I'm going to always do it.

And if I don't have the skill set or the time to learn that skill set, then I should probably send them to someone who's really good at this. You know, not every doctor has to do everything. And so, you know, we do a lot of these. We've done 4,000 plus vasectomy just in the last five years since I've opened the practice. Wow. Well, if you're doing them at five minutes of pop and they could walk in and get it, it's like Botox, you know what I mean? It's really like, it's really that quick and easy. If I were to come to you as a guy and say like, okay, so what are my odds of getting someone

pregnant? Is it like, you know, zero percent, one percent, you know, or there are those people that you hear these crazy stories like, you know, people could take the pill, but maybe they're still going to get pregnant? Is it the same with the vasectomy? Is there like one percent chance you could, a sperm could get through? I'm so glad. you brought this up, Kathy. So when you look at all the different forms of contraception, a vasectomy and we'll say a tubal ligation, which is a much bigger surgical procedure that women can go under, are the two most effective forms of contraception. Now there's the ideal effectiveness rate, and then there's the practical, because here's what happens. Most people

who get pregnant after a vasectomy didn't follow the instructions. So here's here's the thing with the vasectomy. It does not work immediately. So if you think about it, because if we go back to my earlier analogy where I talked about how that Vazdefrens, that tube is really long. And we're putting a roadblock down at mile marker 25. Well, let's say there's sperm, let's say there's sperm already in transit, mile marker 50. They're already passed there.

It takes, and I think we might have even covered this on the last show, the sperm, a guy ejectates today, he actually started making that three months ago. So what we tell guys is, listen, you must continue to use contraception for a minimum of eight weeks. Now, some guys can get clear sooner, but we certainly want to, you know, no guy loves given semen samples. And you also have to aim for a minimum of 20 ejacul. But you've got to do both because I certainly, you know, let's go through a couple scenarios. I get the overachievers like 20. That's two weeks for me. I'm good, bro. And I go, well, good, good for you. Good for you. But all the swimmers haven't gotten to the pool. So you can do a hundred and it's not going to make a difference. You still won't be clear after two weeks. Because it's more time. It's more time. You still need the time is what you're saying, right?

You need the time for all the sperm to get to where the semen is. right and then on the flip side if you have sex four times in two months well you haven't gotten everything emptied out so you also need a certain number of practice round so that's the biggest reason guys and and their partners end up having a pregnancy after a vasectomy is they don't go through that and or the second big mistake they make is they just assume they're sterile after a period of time without getting tested. You have to get tested. It's not optional. We've got to look up. We've got to look under a micro. You have that follow-up for your patients always. They come in and get tested after a certain amount of time. So we go even one step better. Again, let's make it as easy as possible. We sell an

at-home testing kit that has been, that's been proven. So they leave a sample from home. They mail it in. It goes to this company, and this is all they do is read semen analyses. And so, until a guy gets a report back saying, congratulations, we see zero sperm in your sample, assume a pregnancy can happen. Interesting. Oh, I didn't know that. I didn't know any of these interesting facts. I wonder if other guys have.

I mean, are these all the questions that you gathered that are on your video, like all the questions that most guys ask anyway? Am I asking them? Are there ones that I'm forgetting that guys would want to know? Because, you know, I'm not a guy. So, like, you know. Yeah, no, no, they certainly are on my way. website, Kathy, and I think there's a number of other questions. I think related to sort of the

procedure and the recovery, you know, let's just wrap a bow on the question of can you get pregnant after vasectomy? Yes. Once you get to zero, there is a one out of 2000, one out of 2,000, one out of 2,500 chance that the tubes somehow will reconnect. We don't know why. We don't know how. It's one of those things that certainly can occur. So nothing in life is zero. However, you know, 99.95% effective is pretty darn good. Yeah. Now, if, let's say for argument's sake, we do the sample and there's still sperm afterwards, some guys just need an extra couple months and some more practice rounds to clear out the sperm to get sterile.

Some guys, their sample shows sperm, but when we look under the microscope, they're dead. So it's like, you know, sometimes there's some deadbeat, some deadbeat saying about the body. bottom of the pool and they don't want to go. And then rarely I do a procedure and I call a guy up and like, man, I don't know how. I don't know why I've done a shit ton of these, but you're not sterile. And I can't give a good explanation. You know, did we feel something that felt like the tube and it wasn't? Did we feel it and then pull something else up that look like it?

Did a guy have an extra tube? Did he just heal in a weird way where one of them healed back together? We don't ever get that answer. So luckily, you know, it's under 1%, but there are times I've got to have an awkward phone call with a guy and say, hey, man, I'm sorry, you're not sterile. Let me redo this free of charge. And, you know, sometimes we do that in the office if the guy's anatomy is favorable. Other time if the guy was a pretty tough asectomy in the office, we'll just put him under anesthesia. but that's a rare, rare situation. Interesting. Okay, so let's get, it is reversible, right? We know that. And is that something that you do a lot of guys reverse it? So a lot of, no, I would say very rarely do guys come back and ask for a reversal.

Right. And to be, you know, fully transparent. I myself don't do reversals. That's a special level of training you need to do a microsurgical reconnection. Because you imagine how small that tube is. it's one thing to cut it, it's another to perfectly suture it back together in a way that the openings are perfectly aligned. So there are fellowship trained, typically infertility doctors who do that. And I may have had one guy in the past five years out of 4,000, call the office and go,

hey, my wife and I, we got the, we got the, we got the, we got the itch and we want to do this. But it's, it's relatively rare. So I tell guys, listen. whether you've got zero kids or 10 kids, if you're a consenting adult, you're ready, I'm here for you. But that's a personal decision for you to have. But when you do move forward with the vasectomy, I want you to be as certain as possible that you're done or you don't want to have kids. Now, life throws your curveballs, divorces, God forbid, deaths, things come up that can have a guy go, hey, I want to get this reversed. and by all means we can reverse it usually with pretty good success rates. But we want guys going into this with the mindset, like, I'm done.

But isn't the better thing to do, and I'm assuming that this is talked about, like, you know, you do have the option to put some, you know, sperm on ice if you want to just in case so you don't have to reverse it, but you have that somewhere, you know, if needed? It's not, it's, well, compared to having a kid, no. Exactly. Yeah, you know, it's usually, I mean, a couple hundred bucks a year, I mean, at most. So it's, you know, not too bad. All right, some questions for you about like the JIS, you know, afterwards. You know, I had people, I put a question up on my page on like, you know, if you had a Vasectomy, did you have any side effects? Because I just assumed it's like the simple thing, you know, clip, done, you're good to go. I never heard of anything bad happening. But a lot of guys

said things like, oh, you know, I have less ejaculate now. I come less. I mean, is that like in their head because they're over analyzing it or so, or is that something that can't happen? Yeah, it's something physiologically, Kathy, that we'd have a hard time explaining due to the vasectomy. Because if we go back to the anatomy and understanding that again, 95 to 99% of fluid is not coming from the Vaz deference or anywhere where we worked. It's hard to link the two. Now that being said, there's no doubt that as guys age, they tend to have less strong orgasms and that they tend to produce less ejaculate.

They may also be on medications that affect the amount of semen that comes out. So it's certainly possible that there's just confounding things that happen simultaneously around the time I got a vasectomy. But I really have a hard time just to understand the anatomy to definitively say, oh, yeah, the vasectomy caused that. I'd say it's a low likelihood that anything happened during the procedure that would lower the amount of com that came out. Right. Yeah, that makes sense. What about the taste? Because some lady did say, oh, my husband's sperm tasted so much better.

I'm sorry to God. Sorry, I just to ask. Lucky him. Well, because listen, I know, listen, I have made jokes on my show. I'm sorry, like, if someone's taking a lot of medication, like, I swallow. I know, like, come taste different if some guy's taking a lot of medication. You can taste it in the sperm. You can taste things, and calm does taste different depending on certain things.

So, like, does it change the flavor? I can't say by personal experience. However, again, can several hundred million little DNA particles change the flavor of something? Is there something in the DNA that could? I suppose, but more often than not, and I think you answered your own question, is that the characteristics of guys semen very greatly, just based on what he, eats, what he drinks, medications. So again, I have a hard time thinking that just because there's some little swimmers

in there, that that is the main difference it would change. But hey, listen, congratulations to that guy where it tastes better. And maybe that's my new marketing campaign. Totally. That's what you lead with. That's what you lead with. You get a bisectomy better tasting sperm. Last question about like the giz.

Like does it change the color when you don't have those DNA particles or is still going to look exactly the same? It should look exactly the same. Now, certainly rarely guys may have a little blood in the semen right afterwards, but that's a pretty rare occurrence, again, just because of, number one, how minimally invasive the procedures, but number two, how far the blood would have to travel in such a tight little tube to do that. But no, we don't believe there's any real definitive change to the semen,

but certainly everybody is different. the human body is amazing and unique from guy to guy. And there's plenty of things that we don't know in health care. And, you know, these are all great questions that I'd say we don't have definitive answers to. Yeah. And so, like, you're talking to these guys all day long. And a lot of times the guy doesn't want to get the vasectomy, makes his wife get that harder procedure, like you said.

It's a lot worse for the woman to do the tubal thing, right, than a guy to get the vasectomy. But so many guys are so petrified of it. Like, what is their, what is their biggest fear that you hear from them? And then what do you say to them? Like, is it a loss of sensation? What if you clip the wrong nerve and I can't have an orgasm again? Like, what is it that guys are like why they would make their wife go through the procedure as opposed to them getting the vasectomy, which seems like a much easier thing to do?

Yeah, I think, yeah, there's a lot to unpack with this. So let's first start with a tubal ligation, just so guys understand. That is done under general anesthesia and requires a doctor to insert instruments and needles inside your wife's abdominal cavity. And that although when you go to someone who does a lot of them, it goes exceedingly smoothly, quickly, and done as an outpatient, you're poking needles into your wife's belly.

I mean, you can hit major arteries, intestines. It's not a zero risk of dying. And so I really, I would caution any guy, please, please don't make your wife get a tubal ligation. Just go get the vasectomy because, God forbid something awful happened. Now, if a woman is undergoing a C-section, she's undergoing other procedures, and they're in there anyway, sure, by all means, take advantage of that opportunity. But I think it's just, it'd be a shame to have your wife have to go through that.

So now let's go into why do you guys, why are so many guys afraid? Yeah. I think it's misinformation, Kathy. It's all the myths that are out there. And in today's day and age, all it takes is one viral TikTok video on the bisectomy is the worst experience in my life. I've been in pain for six to 12 months afterwards. And this is awful. Don't do this.

And they don't hear the other 500,000 guys who got a bisectomy in this country, Hugo. Oh, that wasn't that bad. So I think it first starts with pain, as most things are with guys in that precious area. and if you go to the right person like our practice here in Nashville, a Posona MD, I can't guarantee anything in my line of work, but I can guarantee we're going to give you as great a service as possible, and it's going to likely be not as bad as you think.

Then secondly, there's all these myths around performance, and that could include changes in ejaculate, orgasm, erections, testosterone. Yeah. And when we go back to what we're actually doing, during the procedure, there is a 0% chance that what we're doing will impact any of that. The nerves that go to erections and the blood vessels are deep in the pelvis nowhere near

the ball sack. We just covered that how the ejaculate comes from somewhere different. Even testosterone, although it's made by the testicles, that gets released directly into the bloodstream from the testicles, and that is not impacted by a vasectomy. So if sex changes after a vasectomy, it's not because you have the vasectomy. And if anything, we've got a number of studies, Kathy, showing that sexual satisfaction improves after a vasectomy because what's sexier to a woman than a guy who takes control of a situation? Right.

A guy who takes something off her plate and goes, thank God I don't have to take these damn pills anymore. I don't have to go out and worry about buying condoms. Think I don't have to worry about whether you pulled out quickly enough. you know, guys forget that sex for a woman starts in between her ears. And so taking charge of that situation. And then both of you having that confidence, like, we don't have to worry about broken condoms, pulling out not soon enough. Like, we're good. As long as we follow our instructions, we're not going to have another kid.

And so those are probably the two biggest reasons are this fear of pain, which is always overblown. And then secondly, that it's going to impact performance. And we just have no data. and understand the anatomy to suggest that either of those things are going to happen. Right. So if they're educated enough, they listen to this episode and they hear, like, how you explained it in the beginning where everything is located, you know, like you said, your little tools that you're using are not anywhere near the nerves that are going to, you know,

give you a heart on or anything like that, right? So nothing will be impacted by this, except you're just not going to get someone pregnant. I mean, that was the other thing. Like a lot of people made jokes on my Patreon when I said, what are the side effects? The side effects. Great. Better sex. I could come inside.

We have cream pies. I mean, all that kind of stuff. You know, they're having more fun because sometimes it's, like, hot for someone to come inside you and not have to worry about it, you know? But there's a certain time in a woman's life where she's got to really worry about that. And if you don't have to anymore, it's great. Oh, absolutely. And it always happens when you're least expecting it and you're least ready for it, right?

And we see a lot of it, you know, couples incorrectly think that just because a woman's going through perimenopause, she can't get pregnant. That's not true. You know, I've had friends who got pregnant while on birth control pills. I think everybody has had a condom malfunctioned. I mean, and listen, you know, again, that decision as to how many kids is a very personal one. But it's the, I think people underestimate the fear and risk of that unexpected kid and the stress that that can cause. I mean, here in Nashville, I mean, you can't afford to buy a house these days.

You know, imagine you're in Manhattan and you're, you know, stuck in a, you know, you can't get another bedroom. Where are you going to put the extra kid, you know, get another car? And, you know, I mean, I think the latest statistic is it's going to cost about $300,000 just to keep a kid alive to age 18. And that's not including. Not a New York City. Yeah, well, that's not New York City. That's not travel sports. It's not private schools.

That's not going on nice vacations. Oh, my God. Poor kid. So, right, right. So, you know, I think people don't, they don't think of their future self. They think of their current self. Go, oh, I don't want to do with pain. Well, to me, the biggest regret I see from people in the biggest pain is the guy who didn't

get it done soon enough. Right. It's a guy sitting out there playing with fire. And then here he is, he's age 50. And maybe he's newly single. And he's like, I'm cool now. And he's out having sex with all his younger women and he's happy. And then she gets pregnant.

Right. Like, dude, dude, you're like. your life just changed big time, you know. And it's not to say there's not going to be a good dad and love his kid, but if really at this point in your life, you're like, hey, kids are really ideally off the table for us. I can't think of a smarter option than just coming down and getting a vasectomy. Yeah, and I love that you, you know, made that whole statement about what the woman has

to do if you choose not to do this little simple procedure. Sorry, you know, women are like having babies. They're growing fucking human beings. in their belly, they're pushing them out a little hole. I mean, come on. Go get a fucking vasectomy. Guys, sometimes could be like little babies when it comes to medical stuff. I don't know.

They're worse than women, for sure. Sorry to saying. You're not going to get any argument to me, and I'm a man, and I'm a less. You know what I mean? Like, go get a vasectomy. It sounds like a very simple thing. I did have, this is my last questions before. I just want to ask you a couple, like, you know, closing questions.

But like the one of the other things that was a side effect, a couple guys said, oh, I had pain in my balls for a certain amount of time. Like, could that be from it? Or is that another thing that you say, you know, maybe it just happened at the same time that they had this pain? Or could they have a little bit of a pain from maybe because they had a different kind of procedure and it leaves a little pain for a little while afterwards? Yeah. I'm glad you brought it up because I was going to. So there is no doubt a buried recovery from guy to guy. Now, most guys by about a week out are pretty good.

There are some men who it can take several weeks. And sometimes that's related to technique or a guy who had a harder to find tube. And sometimes it's not. I mean, I have guys where I did it, it was smooth. And three weeks out, he's calling going, oh, Doc, my balls are sore. What's going on? And there's nothing wrong.

Yeah. Now, there is a real long-term, pain syndrome. It's called post-vesectomy pain syndrome. And this is legitimate. Now, the problem with it first and foremost is you'd think every guy has it because again, all it takes there, you know, one or two guys to go viral and it seems like the whole world's got it, right? Yeah. I can comfortably say that in the last 4,000 men I've treated, I've had one guy legitimately have this. Now, are my rates better because of the technique? Are there maybe guys out there, suffering with it, they didn't tell me. I don't know. The truth's probably somewhere in between,

but there's only been one guy who had it. We don't know why it happens. We haven't been able, at least in studies, to determine, is it related to technique, patient factors, anything at all? It seems to be a random occurrence. We also don't know is the cause related to that buildup of back pressure? Is it a nerve issue? Is it inflammation? We're just really not. sure, but the pain syndrome we're talking about, first off, is exceedingly rare. In the literature, it's about 1%. Okay. So, 99% of guys are going to be just fine.

Number two, we're talking about pain that is present six months down the road. So if you're achy after a couple months, there's still a good chance you're going to keep getting better. So we're talking about a long-term pain syndrome. Number three, not enough urologists know about it. And unfortunately, these poor guys go to somebody who blow the off and instead of recognizing it's a legitimate issue. So there are things that I can do conservatively that can help.

And then sometimes we have to send a guy again to one of those microsurgeons and they either do a vasectomy reversal or they can go in and remove all the small muscles and nerves going to the testicle and that can deaden the pain again without causing any change in performance. So it's a legitimate issue. It's probably underreported, but for your listeners out there, there's a 99% chance you're going to be just fine. And if you're one of those unlucky guys, there's ways to solve it. And if you're out there suffering with it, you just haven't gotten in touch with the right guy who can help you. Right.

And so it can be helped. You just got to find the right person. So is there anything that I didn't ask that you typically get asked that guy should know about, you know, like I said, I'm not a guy. so I don't really know all the questions a guy would ask. No, I think we did a pretty good job covering it all. You know, Kathy, I think the biggest thing that I'm hoping to do is to just build awareness with guys that stop listening to your old co-worker who says you're getting castrated. We're not cutting your nuts off. Stop thinking it's going to affect your bedroom activity.

It is not. And anything you listen to a guy on social media, there's no. reason and no anatomic explanation if that guy's reporting something different. So it's super easy. Take charge. I think the last thing that sort of comes up is, you know, age, you know, and or do I have to have kids? And again, this is one of those where my approach is, listen, if you're a consenting adult and you're a grown ass adult, that choice is yours and is none of my business. So if you're 22 years old and you're like, I came from a fucked up family. I've never wanted kids. I've known about this my whole life. I want this done. There's no way I ever want to do it. By all

means, I should take care of that guy and do a great job. And I think it's a bit patriarchal and arrogant for a urologist to tell that guy, you're not old enough to decide that. Right. The guy's old enough to drink. He's old enough to drive. He's old enough to die for our country. If he wants to get snipped, let him get snipped. Yeah. And so, you know, some you're all just have, oh, you got to have at least one kid or you've got to have two. We've got to be this age. Like, oh, yeah. It's just, and I just, I personally don't agree with it.

Now, listen, studies would support that men's brains are not fully developed until 25. I'm sure your female listeners would probably push that number up to 55. Yeah, yeah, yeah. Like 45, maybe. So, you know, so if it's a guy below the age of 25, and he has no kids. I certainly spent a little bit more time digging and getting curious, but I'm not judging.

You know, I'd say that's the last thing. If you went and you saw urologist and you felt judged, you went to the wrong guy. You know, if you went to a guy and he's going to put you under anesthesia, and he doesn't give you a good explanation as to why or how he always does it, you're not seeing the right urologist. So, you know, there's a lot. I'm not the only guy, but, you know, we've had people flying from Hawaii,

Chicago all over the country, I'm going to see us in Nashville. Because of how you do it? Because that's going to be my last question. Like what is the name of the procedure of how you do it and how do people, if they can't get to you, you know, how do they find someone that does it? Yeah. Well, I'd say, first off, give our office a call. We answer our phones.

We get people worked in quickly. We have opted out of insurance so we don't have to get permission with people anymore. Wow. You know, another question that does come up related is. the cost of things. And what I found when I was taking insurance was probably 80% of guys still hadn't out-of-pocket expense of the whole procedure because they had a high deductible plan. Right.

And on top of that, there was so much headache over trying to estimate the cost, calling the insurance company, hours on the phone. Then people would get pissed off at us because they didn't get the answer they want. And we're like, man, it's not us. It's United Healthcare. Right. And they're waiting for your to get approval, blah, blah, blah. blah. Exactly. So finally said, you know what, we can't keep the doors open battling these people who

want to pay us ridiculous amounts of money for our expertise. So we offer a very transparent, fair pricing for a vasectomy. You can come on down. You get your bisectomy on Friday, go out honky tonking on Broadway, eat some good food, and then fly back home and you're good to go. And people, you know, people are more savvy these days. They're willing to travel for good service because if you compare that to you got a battle insurance, you don't know what your bill is going to be. It's going to take you two months to get in for the console and then another four months to get your procedure, during which you could get your wife pregnant.

Or you come see me in Nashville, know exactly your price, pay it, get it done, have a fun weekend, and then go home. Yeah. And let me tell you, if someone that lives in New York City, I dated a guy of kind of a famous record producer that lived there. And I was like, I can't believe I'm going to Nashville. I just had this idea in my mind of what it was going to be like because I live in New York City, you know, and when I did get to the terminal in JFK, there were people with actual cowboy hats on. I was like, what the fuck is in store for me? Do you know what I'm saying? I'm like, I'm going to the middle of nowhere.

You know, I'm just like ignorant at this point of my life. I got there. Nashville is like the coolest fucking town. It is so, it's like the lower east side. You know what I mean? Like it's very New York city like to me in the main part of town. It's a great restaurant, great food. It's like a really, it's like a great little city. Yeah. And listen, I'm a New Yorker as well, Kathy. And so, you know, we're seeing a lot of transplants from New York. Yeah, I could see why. Chicago, California, no state income tax. And, you know, we got a lot of New York restaurants opening here. I mean, it's been, I love it. It's a fantastic place to be. And it's really become a health care hub. So, you know, we get a lot of people, you know, coming in to see us for these procedures because, you know, guys want to have a good experience. And listen, would you rather go to your uncomfortable guy in six months and get stuck with an insurance bill and have achy balls or come down and see us, be done in 10 minutes with one visit and have a fun weekend? Right. And you can fly home. You don't need that person picking you up. I mean, it just sounds so easy. So it sounds like if people are flying to see you, there's, it's not like this is a common, like, procedure that a lot of urologists are doing.

that you're doing. You know, it's one of those things that, you know, a lot of eurologists will, quote, say that they do it and say, oh, I'm an expert in 50 things. Like, bullshit. You know, we do three things really well at our practice. And if you need other stuff, we're going to send to the people who do a lot of it. You know, I feel that, you know, we should really become experts in what we do, especially stuff like this where, you know, it's funny.

Although it's such a minor procedure, it stresses out my colleagues. so much to do vasectomies. It's funny. Like, they'd rather remove somebody's kidney than do a vasectomy because, well, you know, listen, if you're not confident in your technique and what you do, you've got a young, a young guy who's awake,

who's squirming because your anesthesia's not that great. And the best you could, like, you're not carrying cancer. The best you can do is break even. The best you can do it, the best you can do is hope he doesn't bitch at you. Yeah. And so, you know, for a lot of my colleagues, like, yeah, I'll do it, but I don't love

it whereas I love it. I mean, it's, I've met, I've met good friends this way. I mean, I think half of my wife's friends, I've done a vasectomy on their husband. You know, it's a, it's a time to meet cool guys, you know, my age, I'm in my late 40s and, you know, in the same sort of vein of life. And, you know, we try and give guys a really good experience and make it super easy for them. You know, their wives and partners are welcome to hang out in the room if they want and be there or the guy could drive himself. I love it. So I'm going to give all your information in the description.

What are the two other things, just so that people know if they're there at the same time. What are the two other things you guys specialize in? You said, we do three things like a lot. You know, one is this. Yeah, so one is this. The second is hormone optimization in men and women. You know, we see these guys come in.

They get their vasectomy and then they go, hey, can I check my tea level? with me? Yeah, exactly. I'm a dad and I'm certain to just not feel the same. And then we work on wellness with or without the new weight loss medications, you know, because we realize that a lot of what I treat as a urologist, whether it's erectile dysfunction, bladder problems, even prostate cancer. If you're carrying extra weight and you're not healthy, you know, those, those lead to all your logic conditions. And so, you know, we're really about creating health instead of being reactive and treating sickness. You know, sure, if you got a kidney stone and you want me to take it out, we can do it. But how much more powerful is it if we can get you to lose weight,

optimize your hormones, you know, solidify your contraception. And so you can go live a vibrant life. And I think that's what a lot of would resonate with a lot of your listeners is, you know, they have very active lifestyles, obviously sexually, but I imagine that carries into other parts of their life. Of course, yeah. You know, that's going to all go away one day if you're not taking care of yourself. If you're not optimizing your hormones and you're not watching your waistline and partnering with the right medical practice, like your fun days are numbered, whereas, you know, if you stay young and vibrant and work on all these things, I mean, we have couples having sex in their 80s, you know, so.

So do I. Am I show? You know, and that's, yeah. Yeah. And that's what I want to be doing. Yeah, totally. So that's really what we're about is, you know, treating men and women in a wellness perspective, but from the viewpoint of a urologist because, you know, when you have trouble in the bedroom and the bathroom, you know, that matters to people. And that's going to be sometimes your gateway into a doctor. And so we can handle all of it.

Yeah, I love it. And I feel like those things kind of maybe all, like I think around the time that you're saying, hey, no more kids is the time that maybe you should be. be looking into those other things that you have available anyway, right? It's like, because you're getting a little older. Maybe it is time to look into your testosterone levels and all that good stuff. I believe in hormone optimization. We could do a whole thing on that one day. But thank you so much for calling in and giving guys this information. I think that they needed it. Like I said, as a woman, I was highly uneducated about what the fuck's going on in my body. And I think men are too.

You know, a lot of times we just don't know the simple information, the stuff that you gave in the beginning, I think, is really helpful. And so I'm so glad that you came on and told your story, especially because you're doing it in this way that I think sounds really great. Like, if I was a guy, I'd go to Nashville again to see you. I mean, I think you did a great summary. And, yeah, you know, I'm in my late 40s. And in the past, I'd say, two to three years, even though I've taken really good care of myself, you know, I've started noticing just some subtle changes. You know, you start getting some aches and pains and a little easier to get tired. And you also start sensing your mortality a little bit. You know, my kids are getting older. I got one who's about to graduate high school. And this is the moment, if you haven't, to really invest in your health. Because once you start getting into your 50s and 60s is when diseases typically are going to start showing themselves.

Right. And I don't want to spend my, the next 20s and the next 20s. 20, 30 years in doctor's offices. I don't want to be taking a boatload of pills that have side effects. I'd rather right now optimize my health, optimize my performance. And it starts with solidifying your family planning because anyone who's a parent can attest. When you have a kid, you know, you got to go all in for that kid. You know, you're going to be not sleeping. It's harder to eat right.

It's harder to exercise. you're more stressed. I love being a dad and continue to love being a dad. But now that my kids are a little bit older, it's so much easier to invest in my health than when they were little babies. And so I can't imagine having a kid right now at 48 and being able to invest in my health the way that I am. And so, you know, again, guys and ladies who are listening,

think about your future health, your future health and your future self. You probably have a lifestyle you want to consider. continue and you're one broken condom away from that really rocking your world. So come down and see us. I love it. I love all this information. By the way, I had a baby at 46 and you want to know how I was able to do it. H.R.T. I was in perimenopause. I went on it immediately. I was like there's no way I could be a Karen and have a baby at the same time. So you know what I mean? And that just changed my life. And I was like good to go. You know, felt like I was 35. But with a baby and I'm fine. And I, to me, having a baby at my ages is great because I have all the energy and everything. Because I

believe very much in health and fitness and I've been on hormones and I optimize and I do all that. So I love what you're doing at. I wish I lived in Nashville. I'd come to you.
